You are right, one can't say for sure that it's PAL to NTSC transfer, but most likely this is the reason for field averaging. For Film to NTSC transfers, repeat field pattern is visible even in 4th or 5th generation VHS copies.
Even PAL to NTSC transfers don't have to have field averaging and a repeat field can be inserted every 6th, (instead of 5th for film to NTSC) field, but for lack of effort or competence, it's almost never done. (Abot half of EVP Hindi DVDs have repeat pattern where every 6th field is repeated, indicating a PAL to NTSC or a 25 fps digital to NTSC transfer).
BTW, even Film to NTSC transfers can be field averaged if they really and intentionally want to ruin their encoding.
